‘Your lungs and kidneys are being attacked by your own immune system’. I vividly remember the day I was laying in the ICU in Sioux Falls, SD, being briefed on the condition responsible for the blood I was coughing up. Ever since I was diagnosed with GPA (formerly known as Wegener’s Granulomatosis) I have been captivated by the complexity of human diseases, and autoimmunity, in particular. Surviving a life-threatening condition not only allowed me to better understand the pain and suffering of others. It also taught me the importance of receiving the very best of care and led to a lifelong commitment to becoming a physician who would deliver the highest quality of healthcare.

And so my quest led me to medicine at the six-year M.D. program at the Medical University of Sofia in Bulgaria, one of my country of birth’s most prestigious medical schools. At MU Sofia and its affiliated university hospitals, I had the opportunity to learn about and see patients with a multitude of disease states. During my eight-month internship at Aleksandrovska University Hospital, my interest in Internal Medicine was solidified. The spectrum of diseases seen in the specialty, having the capability to treat various organ systems and deliver comprehensive healthcare from adolescence to geriatrics fascinated me. I felt tremendous fulfillment being at the bedside of patients, clarifying test results for them, discovering interesting physical exam findings and being a part of a team that handles multiple medical problems. I acquired considerable clinical skills and the ability to make sound diagnostic and therapeutic decisions.

The sudden death of my mother-in-law a month before our last exam devastated my wife and me. After a period of mourning, thinking about the meaning of life and self-reflection we decided to return to the USA, where our dream of becoming well-respected physicians started. Not only we felt that America was the best place to further our careers with its advanced medical system and finest training opportunities, but it was the place where with its unrivaled population diversity we felt truly at home.

Even though moving back meant that I needed to balance my studying for the Step 2CK and CS exams with driving for Uber, the challenge I was faced with only reinforced my self-discipline, perseverance and resolve. The invaluable hands-on clinical training I landed in the USA strengthened not only the passion I have for Internal Medicine but my communication and clinical skills, as well as my efficiency. I learned that there was a delicate balance between the thoroughness of a patient’s assessment and the time constraints of the profession, and that a good physician can be efficient without sacrificing the quality of care. It’s a lesson I wish I had learned before my first Step 2 CS exam, where in my desire to perform exhaustive physical examinations and history taking, I was left with no time to wrap up most of the encounters and counsel my SPs.

After rotating with four highly esteemed American physicians, each with a different background, I feel confident in my ability to deliver proficient care to a diverse patient population. I was inspired by the strong and personal relationship Dr. Coleman had with his patients, as well as by his dedication to teaching and evidence-based medicine, amazed by Dr. Lazarevic’s ability to always put a smile on his patients’ face, even the most difficult ones, and the ease with which he analyzed ECGs and diagnosed various conditions, impressed with Dr. Tsyrulnikov’s wound-care expertise. I feel privileged to have had the opportunity to work with these talented physicians who, beyond any doubt, have contributed a great deal to my personal and professional growth.

I feel very lucky that I found a career that I am so enthusiastic about. A residency that selects me will receive an ardent, curious and positive individual who will work tirelessly to serve both patients and colleagues.
